Thank you for volunteering & agreeing to participate in this year's
40th Okinawan Festival!

This year, the waiver form for those that are older than 18 years of age, will be FULLY DIGITAL to fill out, please see the google form below.
If you are under 18 please select the minor waiver form and a parent or guardian signature is required. When filling out the google form, it will ask for an upload at the end upon the completion of all other parts.

Printed waiver forms will be available and accepted on-site at the Volunteers Room (HCC 3rd Floor - Room 316C) or by your booth chairperson prior to your shift; however, submitting the form digitally is highly encouraged.

You will not be able to participate as a volunteer without completing this form.
One (1) waiver is required for the whole festival weekend, so if you are volunteering with other groups please fill out the form only once, and we would like to thank you for your continuous support!
